PARTICIPATION REQUIREMENTS

PIAA ELIGIBILITY

Any student wishing to participate in a PIAA-sanctioned sport for the Iroquois School District must have a PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) dated after June 1 of the upcoming school year they wish to participate (form available under required forms).

  • Physicals completed before June 1 of the current sports season will not be accepted.

  • At the time of the physical, all sports must be signed by the parent on page 2 of the CIPPE. *Subsequent additions would need to be completed using a Doctor Recertification.

  • For multi-season athletes, a Parent or Doctor Recertification is required to participate in the next season.

  • If a student was injured during that season which resulted in the player missing the remainder of the season due to that injury, a Doctor Recertification is required.

  • All Sections 1 through Section 7 (top) must be completed and signed by the parent and student.

  • Insurance information is required in Section 1.

IROQUOIS SCHOOL DISTRICT REQUIREMENTS

The Iroquois School District will require 3 forms for participation;

  • IHS Athletic Handbook (outlines district policies for participation in sport) signed by parent and student participating.

  • Drug and Alcohol Consent Form

  • Release and Waiver of Liability Form

Additional Information

DRUG AND ALCOHOL TESTING CONSENT FORM

The form needs to be signed/dated by both parent and student-athletes as well as the school district employee who witnessed the signatures. This form must be on file before participation.

LIABILITY WAIVER FORM

The Parent or Guardian of participating student-athletes must provide health insurance information, check all potential sports the athlete will participate in, and sign. A district employee must witness the signature. The form must be on file before participation.

IMPACT TESTING

All student-athletes must complete a baseline concussion test prior to participating in PIAA athletics.\

CO-OP Sports

Any athlete who participates in a CO-OP sport must follow all the requirements of both the Iroquois School District as well as the cooperating school.

Record requests will be completed before the start of the season so that information can be shared.
